GLYXAMBI (empagliflozin)
Glyxambi is a combination of empagliflozin (an SGLT2 inhibitor) and linagliptin (a DPP-4 inhibitor) indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It is also indicated to reduce the risk of cardiovascular death in adults with type 2 diabetes and established cardiovascular disease. **Limitations of Use:** It is not recommended for patients with type 1 diabetes due to the risk of diabetic ketoacidosis. It is not recommended for glycemic control in patients with an eGFR <30 mL/min/1.73 m². Additionally, it has not been studied in patients with a history of pancreatitis, and it is unknown if these patients are at an increased risk while using this medication.
How GLYXAMBI Works
Glyxambi utilizes two distinct mechanisms to improve glycemic homeostasis. Empagliflozin inhibits the sodium-glucose co-transporter 2 (SGLT2) in the proximal renal tubules, reducing the renal threshold for glucose and increasing urinary glucose excretion. Linagliptin is a competitive inhibitor of the enzyme dipeptidyl peptidase-4 (DPP-4), which slows the degradation of incretin hormones (GLP-1 and GIP). This increases concentrations of active incretins, stimulating insulin release from pancreatic beta cells in a glucose-dependent manner and decreasing glucagon secretion from alpha cells, thereby reducing hepatic glucose production.
