How SYMPROIC Works
Naldemedine is a peripherally-acting opioid antagonist that binds to mu-, delta-, and kappa-opioid receptors. As a derivative of naltrexone with an added side chain, it possesses increased molecular weight and polar surface area, which significantly reduces its ability to cross the blood-brain barrier. Additionally, it is a substrate of the P-glycoprotein (P-gp) efflux transporter, further limiting its central nervous system penetration. This allows the medication to block opioid effects in the gastrointestinal tract to improve bowel function without interfering with centrally-mediated analgesia.
