Aprepitant is a substance P/neurokinin 1 (NK1) receptor antagonist used in combination with other antiemetic agents to prevent acute and delayed nausea and vomiting associated with initial and repeat courses of highly and moderately emetogenic cancer chemotherapy in patients 12 years of age and older. It is also indicated for the prevention of postoperative nausea and vomiting (PONV) in adults. Aprepitant is not intended for the treatment of established nausea and vomiting or for chronic, continuous administration.

How APREPITANT Works

Aprepitant is a selective, high-affinity antagonist of human substance P/neurokinin 1 (NK1) receptors. It crosses the blood-brain barrier to occupy central NK1 receptors, inhibiting the emetic signals triggered by cytotoxic chemotherapeutic agents. While it has little to no affinity for serotonin (5-HT3), dopamine, or corticosteroid receptors, aprepitant augments the antiemetic activity of 5-HT3 receptor antagonists and corticosteroids to inhibit both the acute and delayed phases of chemotherapy-induced emesis.
