BIDIL (hydralazine hydrochloride)

BiDil is a fixed-dose combination of isosorbide dinitrate and hydralazine hydrochloride indicated as adjunct therapy to standard treatment for heart failure in self-identified Black patients. It is used to improve survival, increase the time to heart failure-related hospitalization, and improve patient-reported functional status. There is limited clinical experience regarding its use in patients with NYHA Class IV heart failure.

How BIDIL Works

The exact mechanism by which BiDil improves heart failure outcomes has not been fully established. It combines two vasodilators: isosorbide dinitrate, which releases nitric oxide to relax both arterial and venous smooth muscle via guanylyl cyclase activation, and hydralazine hydrochloride, a selective arterial vasodilator. Additionally, animal data suggest that hydralazine may mitigate the development of nitrate tolerance, potentially sustaining the efficacy of the nitrate component.
