TWIRLA (ethinyl estradiol)

Estrogen Receptor Agonists
Women's Health Approved 2020-02-14

Twirla is a combination transdermal system containing ethinyl estradiol (an estrogen) and levonorgestrel (a progestin) indicated for use as a method of contraception in women of reproductive potential with a BMI < 30 kg/m². Use is **contraindicated** in women with a BMI ≥ 30 kg/m² due to decreased efficacy and increased risk of venous thromboembolic events. Healthcare providers should also consider the reduced effectiveness of the patch in women with a BMI ≥ 25 to < 30 kg/m² before prescribing.

How TWIRLA Works

Twirla is a combined hormonal contraceptive (CHC) that lowers the risk of pregnancy primarily by suppressing ovulation. The combination of the estrogen ethinyl estradiol and the progestin levonorgestrel alters the cervical mucus (to inhibit sperm penetration) and the endometrium (to reduce the likelihood of implantation), though ovulation suppression is the primary mechanism.
