POMALYST (pomalidomide)
Pomalyst is a thalidomide analogue indicated for adults with multiple myeloma (MM), used in combination with dexamethasone, who have received at least two prior therapies (including lenalidomide and a proteasome inhibitor) and demonstrated disease progression on or within 60 days of completing their last therapy. It is also indicated for adult patients with Kaposi sarcoma (KS) that is either AIDS-related (following the failure of highly active antiretroviral therapy) or in patients who are HIV-negative. The KS indication is granted under accelerated approval based on overall response rate.
How POMALYST Works
Pomalidomide is an immunomodulatory agent with antineoplastic and antiangiogenic activities. It functions by binding to cereblon, a component of the cullin ring E3 ubiquitin ligase enzyme complex. This binding triggers the ubiquitination and subsequent proteasomal degradation of specific transcription factors, namely Ikaros (IKZF1) and Aiolos (IKZF3). In multiple myeloma, this leads to direct cytotoxicity (including in lenalidomide-resistant cells) and apoptosis. Additionally, pomalidomide enhances T cell- and natural killer (NK) cell-mediated immunity and inhibits the production of pro-inflammatory cytokines such as TNF-α and IL-6.

In females of reproductive potential, obtain 2 negative pregnancy tests before starting POMALYST treatment. • Females of reproductive potential must use 2 forms of contraception or continuously abstain from heterosexual sex during and for 4 weeks after stopping POMALYST treatment [see Contraindications (4) , Warnings and Precautions (5.1) and Use in Specific Populations (8.1 , 8.3) ]. POMALYST is only available through a restricted distribution program called PS-Pomalidomide REMS [see Warnings and Precautions (5.2) ].
